Individual-Centred Planning – we will ask patients about their needs, strengths, and personal goals for an accurate determination of the treatment they should receive

Intervention and Assessment – a patient’s challenging behaviour will have reasons and triggers behind it, so we will work to find out the causes and teach every patient how to cope with it and change their mindset and reactions

How to make use of the Positive Behaviour Support Plan

To make use of a Positive Behaviour Support Plan, a person must first see if they are qualified for NDIS assistances by contacting the local NDIA office in Wollongong and getting their plan approved. If there is interest in getting connected to service providers, these are the requirements that need to be met:

  • Must be living in Australia and is an Australian citizen, Permanent Visa Holder, or a Special Category Visa Holder; and

  • Must be between the ages of 7 and 64 years old.

Additionally, applicants must also have one of the three following criteria apply to them:

  • Must need current support to reduce needs in the future;

  • Must need support from another person because of a permanent and significant disability; or

  • Must use special equipment because of a permanent and significant disability.

Once the application has been approved, a client can start giving the provider all the information they need to build the right NDIS plan, which will include the Positive Behaviour Plan under the Improved Relationships category. NDIS participants are required to undergo an assessment of their disability and present condition, where it is key for them to share information about their everyday routine, their emotions, their behaviour, and their feelings on an average day, as well as what needs improvement in their lives.

Apart from undergoing the assessment, clients should also disclose what treatments and medications they are presently undergoing, as well as other existing supports you are receiving. If they are already under the care of a physician, therapist, or counsellor, it would be helpful to have these healthcare workers the clients’ physical and mental condition and provide a letter of recommendation.
