Administering NDIS Assessments
Although NDIS is catered to helping thousands of Australians with a disability, the support plan provided by the NDIS is provided to eligible participants. Through NDIS Assessments, participants ages 7 years and older are recommended to undergo independent assessments to start with the process and make sure that the support plan is in line with the participant's condition, goals, and needs. Once the planning process is administered, the NDIS Assessment can help in determining significant changes in the participant’s condition.

NDIS Assessments Tools
To assure that the support plan is in line with the participant's condition, goals, and needs, NDIS offers assessment tools that are internationally acknowledged to help the healthcare professionals determine the condition of the participants and create a suitable support plan. Each assessment tool is designed based on the participant's age, instead of their condition.
Vineland 3
The primary tool used to evaluate adaptive behaviour is Vineland 3. It is a tool that can help with the diagnosis of developmental and intellectual disabilities. It also provides essential information to develop educational and treatment plans. When administering Vineland 3, it can measure the participant’s social and personal skills that are essential for daily living, such as Communication, Daily Living Skills, Socialisation, and Motor Skills. This assessment is best suited for ages 9 years and below.
Brief Cognitive Status Exam
Brief Cognitive Status Exam is a brief and dependable test specifically designed to assess the global functioning of individuals with dementia, traumatic brain injury, mild intellectual disability, or suspected Alzheimer’s disease.
World Health Organisation Disability Assessment Schedule (WHODAS) 2.0
Directly related to the International Classification of Functioning, Disability, and Health (ICF), WHODAS 2.0 is a standardised assessment tool designed to assess cognition, mobility, self-care capabilities, socialisation, performance, and participation in household and work of the client. The assessment tool is internationally acknowledged and applicable to varying disabilities and cultures for adult participants.
Lower Extremity Function Scale (LEFS)
Designed to assess the function of the lower limb, LEFS is an assessment consisting of 20 questions regarding an individual's struggle with accomplishing daily activities. A lower LEFS score indicates significant disability.
Wechsler Adult Intelligence Scale-IV (WAIS-IV) or Test of Premorbid Functioning (TOPF)
One of the most advance and generally used tests administered to adults to assess cognitive ability is WAIS-IV, or now known as TOPF. This assessment tool can determine cognitive functioning of adults before the onset of injury or illness.
Craig Hospital Inventory of Environmental Factors (CHIEF)
CHIEF is designed to assess psychosocial factors that can affect an individual's health condition, disability, and function that can hinder performing daily activities. The assessment consists of 25 questions rating environmental barriers for adults when performing in their residence, workplace, or the community. The assessment considers environmental areas including attitude and support, service and assistance, physical and structural, policy, work, and school.
Health of the Nation Outcome Scales (HoNOS)
A clinically approved assessment tool, HoNOS, is designed to assess the health condition and social function of individuals with severe mental illness. HoNOS is used to assess the behaviour, impairment, symptoms, and social participation.
Participation and Environment Measure- for Children and Youth (PEM-CY)
The PEM-CY is created for children and youth, ages 5 to 17 years regardless if they are able-bodied or specially abled. The newly created tool is designed to help parents and healthcare providers thoroughly understand a child's participation in their different environments such as their residence, school, and community.
Paediatric Evaluation of Disability Inventory Computer Adaptive Test (PEDI-CAT original and ASD version)
Designed for children and youth with various physical or behavioural conditions, PEDI-CAT is administered through a questionnaire filled out by the participant's family or carer to assess daily activities, mobility, social and cognitive functions, and sense of responsibility. A tailor-made PED-CAT has also been created for children diagnosed with ASD.
